You can be a shareholder or a beneficiary of Assam Healthcare Cooperative. Both shareholders and beneficiaries enjoy equal welfare benefits, but shareholders are entitled to vote in the annual general meeting (AGM) of the society and are eligible to be elected or nominated to the board of directors.
BENEFICIARY APPLICATION
Fee: Rs. 500.00 (LIFELONG) per individual.
The membership fee may be paid to A/C No 37171614127 / IFSC SBIN0007976, SBI, Guwahati-3.
You will need a recent passport-size photo for the ID card.

Alternatively, email your (and any family member’s) name, address and contact phone number to contact@assamhelthcare.coop or send by WhatsApp (90852-28863). Don’t forget to attach the passport photo and the transaction receipt.
We will post the membership card to you, or you may collect it from our office at Guwahati Club.
You may also Download Beneficiary Member Application Form with Info Leaflet, fill and submit it personally at our office and pay the fee in cash.
