Join as a beneficiary of Assam Healthcare Cooperative and enjoy all the welfare benefits.
Fee: Rs. 500.00 (LIFELONG) per individual.

  1. Pay the fee by scanning the UPI QR code (or pay to A/C No. 37171614127 | IFSC SBIN0007976, SBI, Guwahati-3).
  2. You may apply for up to 4 additional household members in addition to you (in total 5).
  3. Save the payment receipt to upload.
  4. Save a recent passport-size photo for the ID card for each applicant to upload.
  5. You are now ready to apply online by clicking the APPLY button below.

Shareholders

You may be a shareholder of the Assam Healthcare Cooperative Society, if you are inspired by its mission and would like to actively participate in its activities. As a shareholder, you are entitled to participate in the society’s Annual General Meeting (AGM) and be eligible to get elected to its Board of Directors, the executive decision-making body of the Cooperative. Please contact us if you like to be a shareholder of the society.
